Royal City Local Staffing Investigation Form

Please use this form to fill out your detail information. This form will allow you to type from your keyboard all the information our Local Office will require, when we investigate issues regarding how CPC is handling your staffing schedule.

Note: Once you have completed this Staffing Investigation Form, print a copy for yourself as you cannot save the data typed into this form. Then either fax or mail to our Local for review as soon as possible. Should you have any questions, please telephone us or email us your concerns.

Grievance Investigation Form

This online grievance form is now available for our Members who have been aggrieved. Please fill in as much information as possible about your situation.  Once you are completed, save a copy for yourself and then fax or mail to our Local’s Office (not Regional) for your dispute to be reviewed and filed by our Grievance Officer. You’ll in turn recieve a letter with a Grievance number as confirmation that our Local has processed your grievance.

Remember you have 25 days to file with us from the 1st day you knew about your dispute.

Appendix LL – Request for Assistance Form

This form is used to request assistance from Mgt. as a result of incurring overtime on your own route. If you have met the requirements of Appendix LL (please see our Urban Collective Agreement), you can submit this form.

If you have questions on this matter, ask or email your Local for clarification.

Appendix K -Request additional Relay Box Form

For LetterCarriers who are experiencing overburdening situations on their route, this form is filled out and submitted to Mgt. for a request to have additional Relay Box(s) added to your route. Please provide a copy to the Local as well as a copy to yourself.
